___________________________ (Owner) of ____________________________ (horse's name) authorizes Double B Acres to transport said horse as follows:

To transport above horse to receive emergency/normal veterinary care.

To transport above horse to Owner's home.

To transport above horse to receive farrier care.

In addition, Owner releases Double B Acres from liability arising from any harm, injury or death that may occur during transport as specified by state law as a reasonably inherent risk.

WARNING: Under Missouri law, an equine professional is not liable for an injury to or the death of a particiant in equine activities resulting from the inherent risks of equine activities pursuant to the Revised Statues of Missouri.

IN WITNESS WHEREOF, the signature of both parties constitutes a binding agreement, subject to the above terms, and will remain so throughout the duration of training period.
